It's Minesweeper, except you shoot at the tiles with a gun and hope it all doesn't blow up in your face. Originally released at BlenderArtists.org in late 2009, and added to GameJolt In 2011.

It's available as a .exe file and as the source blender file (which requires an old version of Blender -- v2.49). You can find both of them over at GameJolt.
